Wellness Assessment and Plan Paper Instructions
The purpose of the Wellness Assessment and Plan Paper is for you to be able to objectively explain issues that arise from being a professional counselor such as personal wellness and self-care strategies (Syllabus MLOs: C, I and Module/Week 7 LOs: 2, 3, 4).
Having learned last module/week about the incidence and risk of impairment for professional counselors, this module/week you will assess your own risks and develop a personal wellness plan. Begin by taking the following assessments:
- Stress Reactions Inventory
- Self-Care Assessment, Self-Care Life Pie Worksheet, and Self-Care Social Support Worksheet.
- What were your results of the Stress Reactions Inventory? In what ways do you see your results reflecting your current life?
- What were your results of the Self-Care Assessment, Self-Care Life Pie Worksheet, and Self-Care Social Support Worksheet? In what ways do you see your results reflecting your current life?

Divide your paper into the sections listed below, addressing each question found in the sections. Your paper must be in current APA format (including a title page as well as an abstract and reference page). The abstract must provide a brief, comprehensive summary of the contents of the article in 150–250 words. Note: The title page, abstract, and reference page do not contribute toward the page requirement.

This section must be 2–4 pages.
Wellness Strategies
Looking at your results of the afore-mentioned assessments:
- What areas of your life need more or less attention?
- What changes do you need to make?
- What steps are you willing to take to reach your goal of implementing needed changes?
This section must be 2–4 pages.
Step 1: Submit your paper via the SafeAssign draft link by 11:59 p.m. (ET) on Thursday of Module/Week 7.
Step 2: Wait until the link indicates that SafeAssign has analyzed your paper and prepared a report. If you are uncertain about this step, see the website on SafeAssign in the Reading & Study folder of Module/Week 2.
Step 3: Open the SafeAssign report. (Again, if you are uncertain, see the directions on using SafeAssign referenced above.) Evaluate the report. You will notice that SafeAssign often matches on headings, titles, and references. If you see matches on entire sentences or paragraphs that are not enclosed in direct quotation marks, then you have unintentionally plagiarized. You will need to go back and re-write those sections in your own words.
Step 4: Save your paper as a Microsoft Word document using your name and assignment title (e.g., Doe_J_assignment_1).
Step 5: Submit the final paper using the second assignment link.
Submit your final assignment by 11:59 p.m. (ET) on Sunday of Module/Week 7.
